Building continuity of care through connected Medication Reviews

When we talk about person-centred care in aged care, continuity is essential. Residents often move between settings, home, aged care facilities, hospital, and at every step, medication safety depends on clinicians having access to up-to-date, trusted information.

Yet too often, medication reviews don’t move with the resident. Residential Medication Management Reviews (RMMRs) and Home Medication Reviews (HMRs) are vital tools for ensuring safe, effective use of medicines, but when they remain locked in PDFs or emails, critical recommendations get lost in transition.

This gap has real consequences:

  • Delays in care and treatment decisions
  • Risk of duplication or conflicting therapies
  • Errors that could have been avoided
  • Frustration and extra work for busy clinicians
  • Loss of trust with residents and their families.

Stronger Standards require stronger systems
The strengthened Aged Care Quality Standards rightly call for safer, more person-centred, and well-governed care. But standards alone don’t deliver safety, systems do.

Meeting these expectations means thinking beyond compliance. It means supporting care teams with reliable access to the right information at the right time.

A practical, purpose-built solution
At Meditrax, we’ve worked alongside aged care providers for nearly three decades, supporting better medication governance through expert consulting, policy development and education.

We saw the need for a more connected, real-time approach and worked with clinicians, consulting pharmacists and sector partners to build it.

Our new platform is Australia’s first to enable direct, secure upload of RMMRs and HMRs to My Health Record. This means clinicians in any setting can see updated medication recommendations when they need them, reducing duplication, minimising errors, and supporting safer, faster decisions.

Key features designed for aged care workflows include:

  • Secure integration with Webstercare MedCare™ eNRMC platform
  • Offline access for reliable use in facilities with variable connectivity
  • A purpose-built dashboard with practical insights to support audit readiness and medication governance
  • Automated reporting and streamlined workflows to reduce time spent on admin.

Proven results in the real world

Early results show measurable time savings across the entire RMMR and HMR visit lifecycle, including preparation, in-facility work, and post-visit follow-up. Automated medication profile syncing has removed the need for manual data entry during visits, while an improved user experience supports faster, more efficient operations on-site.

Post-visit workflows, including My Health Record submissions, can now be completed in under 30 seconds with automated reporting and claims. The platform also provides instant access to business intelligence and AI-powered insights to support faster, better-informed clinical decisions.

A shared responsibility
Change in aged care doesn’t happen in isolation. It takes leadership, collaboration, and the willingness to modernise the systems behind the scenes to deliver on the intent of person-centred care.

We’re extremely proud of this innovation, because it supports providers to close long-standing gaps and deliver safer, more consistent care.

We invite aged care leaders to see what connected medication management can look like in practice.
